When a parent’s health becomes more complicated, knowing who holds the whole picture matters. We bring their history, results and care together, helping your family plan ahead.

What we assess

We review medical history, current treatment and everyday health alongside strength, walking, balance and bone density.

Assessments may also include blood pressure over twenty-four hours, memory, mood, hearing and vision, according to clinical need.


Your care

For stable health needs, we recommend a primary care plan. Where several doctors or hospitals are involved, we gather and review their records before recommending the next steps.

A named GP provides everyday care, with specialist involvement where required. Where appropriate, an exercise physiologist supports strength and balance.


Between visits

Your coordinator helps organise care and provides a point of contact for the family. Home care and visits are available according to the membership and service arrangements.


Membership options

Constant provides a named GP, an annual assessment and coordinated records. Absolute adds practical longevity care and care at home.

The Family Health Office offers a separate route to coordinated care across generations.
