TERMS AND CONDITIONS FOR SURGICAL PROCEDURES
These Terms and Conditions of Business ("Terms and Conditions") apply to the provision of services by Reborne Longevity Limited (Company No. 16123585), registered at 9 Queen Anne Street, London, W1G 9HW (“Reborne”) in connection with surgical procedures undertaken on a day case basis. In the event of any inconsistency between these Terms and Conditions and other materials provided, these Terms and Conditions shall prevail.
1. DEFINITIONS AND INTERPRETATION
The following definitions apply throughout these Terms and Conditions:
Balance: The portion of the fee for a procedure that remains unpaid by the due date.
Booking Confirmation: The form outlining the procedure, fee, and payment deadlines.
Clinician: A nurse, doctor, or surgeon involved in patient care.
Consultation: A discussion with a clinician regarding the proposed procedure.
Consent Form: The form signed by the patient prior to the procedure.
Fee: The total cost of the procedure.
Patient: Any individual receiving care at Reborne aged 18 or older.
Procedure: A day case surgical intervention arranged by Reborne.
Surgeon: A GMC-registered surgeon, often with RCS membership or equivalent.
2. REBORNE’S OBLIGATIONS
Reborne will ensure all procedures are performed by qualified and indemnified clinicians, provide suitable facilities, and supply comprehensive pre-operative information to the patient.
3. PATIENT’S OBLIGATIONS
Patients are responsible for understanding the information provided, signing necessary consent forms, complying with all pre and post-operative instructions, and making timely payments.
4. CLINICAL RELATIONSHIP AND RESPONSIBILITIES
Surgeons are independent practitioners legally and professionally responsible for the procedures they perform. Reborne is not liable for clinical outcomes but will ensure that all surgeons are appropriately qualified and indemnified.
5. PAYMENTS
a. Self-Pay Packages: Patients will receive a detailed quote including all standard services.
b. Surgeon Billing: Reborne typically collects on behalf of the surgeon. Direct payments, if required, are communicated.
c. Booking & Balance: A consultation fee due separately. A £500 deposit to secure a surgical date. Full balance due 6 weeks before the procedure.
d. Medications & Extras: Post-operative prescriptions, garments, and additional items are charged separately.
e. Additional Investigations: Any extra diagnostics or tests not initially quoted are billed separately.
f. Payment Methods: Reborne accepts bank transfer, credit/debit card, and third-party finance.
g. Finance Options: Reborne is not a Financial Intermediary. Finance packages are offered via third party providers only.
CANCELLATION POLICY
6.1 Cancellation by the Patient:
Consultation fees are refundable if cancelled at least 48 hours in advance.
Patients have a 14-day cooling-off period post-consultation for a surgical booking with only the consultation fee retained.
Cancellation fees depend on timing and are listed below:
a. More than 6 weeks Consultation + £200 Pre-op fee and any diagnostic fees
b. 6–4 weeks As above + 20% of the total fee
c. 4–2 weeks As above + 50% of the total fee
d. Less than 2 weeks 100% of the total fee plus any diagnostics fees
6.2 Cancellation by the Surgeon or Reborne:
a. If deemed medically unsuitable or unsafe, Reborne will issue a full refund minus consultation and pre-operative costs.
b. Cancellation due to undisclosed medical history or non-compliance may incur full charges as per 6.1.
7. POSTPONEMENT POLICY
7.1 Patient-Initiated Postponement:
Permitted for up to 12 months for medical reasons (24 months for pregnancy).
Charges apply depending on notice period:
e. More than 6 weeks NA
f. 6–4 weeks 20% of total fee plus any diagnostic fees
g. 4–2 weeks As above + 50% of the total fee
h. Less than 2 weeks 100% of the total fee plus any diagnostics fees
The fee must be paid before rescheduling.
7.2 Reborne-Initiated Postponement:
a. If needed due to clinical or facility reasons, new dates will be offered within 12 weeks.
b. If not possible, a refund may be offered minus consultation and test costs.
8. RE-ADMISSION POLICY
a. In rare cases where outcomes do not meet expectations, revision surgery may be offered at the surgeon’s discretion.
b. If revision is clinically indicated within 24 months and is aligned with the expected result agreed during the consultation, Reborne may waive surgical/hospital costs.
c. If the result was affected by lifestyle, non-adherence, ageing, or missed reviews, full fees may apply.
d. Revision surgery must be scheduled within 12 months once agreed.
e. No revision will be offered for previous third-party surgeries.
f. Postponed/cancelled revisions within 4 weeks are subject to full fees.
g. For procedures involving implants, re-admission for rupture or capsular contracture within 3 years may be covered.
h. Manufacturer warranties apply only if remedial surgery is carried out at Reborne.

c. A written complaint must be made within 12 months of the original appointment or procedure and the process set out in the Complaints Policy will be followed.
10. CONFIDENTIALITY AND DATA PROTECTION
Reborne complies with GDPR. Patient data is shared only as necessary for care and will not be disclosed without consent, unless legally required.
11. VARIATION
No variation to these terms is valid unless agreed in writing and signed by a Director of Reborne.
12. GOVERNING LAW
These terms are governed by the laws of England and Wales. Disputes shall fall under the jurisdiction of English courts.
13. ADDITIONAL COSTS
Costs for additional consultations, tests, hospital admissions, or medications beyond the original quote are chargeable to the patient and must be paid in full in advance.
