Concierge GP in Marylebone. The Constant membership
Constant is Reborne's concierge GP membership. You have a Reborne physician who knows you, a coordinator who runs your care through the year, and a concierge who arranges everything, inside a CQC-registered hospital at 9 Queen Anne Street, Marylebone.
It begins with an unhurried assessment of where your health stands, and it continues as GP appointments when you need them, message contact between appointments, home visits within the M25, your regular treatment kept under review, age-appropriate screening and vaccination, and referrals arranged and followed, including into hospital care. Membership is from £1,250 a month. Non-members are welcome to begin with a conversation.

THE CONSTANT MEMBERSHIP
What you get
Your year begins with an unhurried assessment. We gather your history from every provider you have seen and bring it into one record, which the Chief Medical Officer reviews. Your physician examines you, and we run the Reborne blood panel, including ApoB and Lp(a), together with blood pressure over twenty-four hours, a heart tracing, lung function, grip strength, vascular stiffness, bone density and body composition, and a liver scan. You receive a written formulation of where you stand and what we will do about it.
Through the year, your Reborne GP team is there when you need it: appointments across the year, message contact between appointments, a share of the triage rota every day of the year, and home visits within the M25. Stable conditions are managed as your primary care. Hormonal health is investigated, treated and reviewed. Your regular treatment is kept under review and renewed. Referrals are arranged and coordinated, including into hospital care, and your records travel with you. Every inclusion is subject to your doctor's clinical judgement.
YOUR PHYSICIAN, COORDINATOR AND CONCIERGE
How it works
Three people look after you. Your physician is a Reborne GP who examines you, reads your results with you and takes the clinical decisions; the same doctor, year after year, so nothing has to be explained twice. Your coordinator keeps the year moving: appointments, results, follow-ups and the records that come back from specialists, all brought together in one place. Your concierge arranges the practical side, from bookings and home visits to referrals and second opinions, so that you make one call and the rest is handled.
All of this happens inside a CQC-registered hospital at 9 Queen Anne Street. When something needs a specialist, imaging or a procedure, your physician refers you within the building or to the right consultant elsewhere, and stays involved.

THE FIRST CONVERSATION AND THE YEAR AFTER IT
What to Expect
It starts with a conversation. You tell us what matters to you and what you want from your doctor, and we tell you plainly whether Constant is the right membership or whether something lighter or deeper would suit you better. There is nothing to prepare and no obligation.
If you join, your coordinator books the assessment at the townhouse at 9 Queen Anne Street: time with your physician, the blood tests and measurements, and the scans, arranged around your diary rather than ours. You then sit down with the same physician to go through the results and receive your written formulation. From that point the membership runs in the background of your life. When you are unwell, you message or call and you are seen or spoken to by a doctor who already knows you. When something needs more, the referral is made and followed, and the result comes back into your record. Nothing is handed over as a printout and left with you to chase.
WHO JOINS CONSTANT
Who It Is For
Constant suits people who want one doctor, one record and one point of contact for the whole of their health, without a larger programme around it. Five situations bring most of our members to it.
You may have a GP in name but see a different doctor each time, explain your history again at every appointment, and wait days for a slot when you are unwell. Constant gives you one Reborne physician who has examined you, read your results and keeps your record, and who you can message or see when you need to. The relationship is the product.
Raised blood pressure, cholesterol, thyroid, diabetes, hormonal changes: conditions that are stable but need watching and adjusting, year in, year out. Your Reborne physician manages them as your primary care, with the twenty-four-hour blood pressure readings, the blood tests and the reviews built into the year, and your regular treatment kept under review. If something changes, the referral is made from inside a hospital.
If you divide your time between London and elsewhere, the problem is usually continuity: three doctors in three countries and no one holding the thread. Your Reborne physician holds it. Your record is in one place, consultations can be remote when you are away, referrals are arranged wherever you are, and the concierge deals with the practicalities of care abroad so that you come back to a doctor who knows exactly where things stand.
Parents, children and grandparents can each hold a Constant membership with the same team, so there is one number to call and one coordinator who knows the whole family. Records are kept together where you want them to be, and the concierge handles the arrangements for everyone, from a child's vaccination to an older parent's referral.
You want your blood pressure, cholesterol, blood sugar and the screening due at your age looked at properly every year, by a doctor who then acts on what is found, without committing to a larger longevity programme. Constant does that: the annual assessment, the written formulation, and a physician who follows it through.

A CONVERSATION FIRST
Is Constant right for me?
Constant is the right membership if what you want is a doctor who knows you, your record in one place, and someone to arrange everything, delivered from a CQC-registered hospital rather than a clinic. If you want more than that, with frequent data, specialist consultations and a programme of optimisation built around you, Prevent and Optimise and Absolute begin where Constant leaves off, and the same team will tell you plainly which fits.
The simplest way to decide is to talk to us. Message the concierge on WhatsApp, call 020 3839 5051, or book a first conversation below. There is nothing to prepare and no obligation.
